Which forms are included
Tattoo and piercing forms are included in the base package. Additional forms such as tooth jewelry can be added optionally. The workflow always stays the same: clean, traceable and safe.
Tattoo form
Consent, medical information and signatures – structured and complete.
- Client data, contact details, date of birth
- Medical questions and relevant risks
- Consents and studio texts (loaded dynamically from settings)
- Event-mode ready (conventions / walk-ins)
- Design, stencil and reference images stored per consent
- Signature(s) captured directly in the form
- Touch-ups documented with a separate signature and date (legally separated)
Piercing form
Piercing-specific questions including special cases – without forgetting anything.
- Piercing location, previous piercings, complications
- Relevant medical information (e.g. blood pressure, allergies, etc.)
- Event-mode ready (conventions / walk-ins)
- Option to record a rejection with documented reason
Tooth jewelry form
Tooth-jewelry-specific questions with clear risk disclosure and clean documentation.
- Dental status (e.g. cavities, gum issues, dental restorations)
- Relevant medical information and allergies (e.g. adhesive, latex, nickel)
- Risk information such as enamel damage or detachment
- Consent and signature specifically for tooth jewelry
Protection built into the workflow

Validation & required fields
If important information is missing, it won’t be “saved anyway” – the workflow guides the user correctly.
- Required fields: name, date of birth, consents, etc.

Under-18 workflow with blocking
For minors, nothing is waved through. The system enforces the correct process.
- Service-specific minimum age (e.g. piercing from 12, tattoo from 18). The age limit is defined per form and determines what is allowed.
- Guardian / parent details are mandatory
- Signatures and uploads are mandatory
- Submission is blocked until all required elements are complete
